Does anyone know how the simultaneous dual boarders did it in the day before the official combined programs (e.g., Wilford Spradlin)? Is that still a possibility today?

The people I know did one residency (typically internal medicine) and then did a second residency (psychiatry) afterwards. You can still do this, and I know 2 people who have done this recently (within the past 10 years). The upside is you get to pick where you do your residency for each program (both people I know did them at the same place though). The downside is that it will take you an extra year or so to complete the two residencies compared to a combined program.
